New York State Certificate Reporting

The New York State Education Department (NYSED) has two course requirements for licensure:

Child Abuse: New York Mandated Reporter Training - a one-time requirement to be completed prior to initial licensure.

Infection Control and Barrier Precautions - to be completed every other renewal period (every four years).

In order to report these courses to the NYSED, you will need to click the report link located next to each course certificate after successful completion and then provide the required information as follows:

• Last four digits of your social security number. (If you do not yet have a SSN, then enter all zeroes in the field instead)
• Date of birth
• License number *for those already licensed
• Profession

Please note: CEUfast does not store any of the required information. It is held only long enough to be sent securely to the NYSED via encryption. Once the transmission has been completed, the information is no longer available in the database for user security.
