The Utah Board of Nursing requires LPNs to renew their license before January 31 of even numbered years. While some states set license expiration dates based on the licensee’s initial licensure date, Utah does things differently. Under Utah’s license renewal system, all LPN licenses expire as a group on January 31 every two years on even numbered years.
During the two years under license, LPNs must fulfill one of three continuing education requirement options. LPNs in Utah are not required to actually report CEs, but must check off on completing one of the requirement options. After completing all continuing education requirements, licensees must submit a license renewal form with a $58 non-refundable license renewal fee.

The Utah Board of Nursing does not approve individual CE providers. However, the Board accepts any CE courses from any provider who is accredited by the American Nurses Credentialing Center's Commission on Accreditation (ANCC).

Start TodayIf you hold a secondary license, it will renew at the same time as your primary license when you renew online. Use your primary license number when you log into the online system and both licenses will be processed together. However, if you hold more than one primary license then you have to complete the renewal process separately.
Before you renew your license online, you should receive materials in the mail from DOPL. You will use those renewal materials when you renew online.
Step 1. Complete all continuing education requirements.
Step 2. Enter your license number, renewal ID and last four digits of your social security number to log into the online renewal system.
NOTE: If you do not already have a renewal ID, you may request one on this page.
Step 3. Once you have logged into the renewal website, you will need to verify and update your license information.
Step 4. Read and answer the renewal questions carefully. You must also certify that you have completed or will complete all renewal requirements, including CE hours.
Step 5. Pay the renewal fee of $58.
Step 6. Once you complete the renewal process, you will be given a Certificate of License Renewal. Print this out and use it as a receipt for your records.

Step 3. Read through the form carefully and answer four certification questions. You may have to attach some additional documentation, such as a copy of your valid U.S. passport. Sign the Affidavit.
Step 4. Pay the fee of $58 in the form of money order or check. Make the check or money order out to “DOPL.” NOTE: Do not send cash.
Step 5. Mail all fees, forms and documentation to DOPL at PO Box 146741, Salt Lake City, UT 84114-6741.
