Must have:
(A) completed 20 hours of continuing education approved by the Director by rule;
(B) participated in at least four peer reviews;
(C) submitted individual practice data;
(D) maintained current cardiopulmonary resuscitation certification; and
(E) filed a timely certificate of birth for each birth at which he or she was the attending midwife, as required by law.
Physical Therapists are required to renew their license biennially on September 30th of the even-numbered years. Licensees must have completed 24 hours of continuing competence activities to renew.
Physical Therapist Assistants are required to renew their license biennially on September 30th of the even-numbered years. Licensees must have completed 16 hours of continuing competence activities to renew.
